Contact your local Aging and Disability Resource Center to begin your IRIS enrollment. Your ADRC will help you choose an IRIS Consultant Agency, such as First Person Care Consultants. If you choose FPCC as your IRIS consultant agency, our intake and enrollment specialist will assist you with choosing your IRIS Consultant. Once your IRIS Consultant is chosen, they will assist you with enrollment and ongoing support in the IRIS program. You can find your local ADRC here. Learn more about the enrollment process here.
The IRIS service definition manual details the long-term care services and supports covered by the IRIS program and qualifications providers must meet to provide such services and/or support. Your IRIS Consultant will assist you in determining services and supports to meet your long-term care goals and needs

The services available to you through First Person Care Consultants can help you achieve and maintain your highest levels of independent living through the IRIS program. Our IRIS consultants understand and support you as you make your own choices about the long-term care services you need. Your IRIS consultant will assist you in the following ways through your journey of self-direction in the IRIS program:
1.) Developing personal self-directed service plans
2.) Talking with you about your long-term care choices
3.) Completing annual IRIS education requirements and forms
4.) Completing annual required screenings
5.) Supporting self-direction for Medicaid-funded support
6.) Resolving IRIS program-related issues and conflicts
